Nurse's Notes

Dear Parents:

New York State law requires a health examination for all students in Grades 1, 3, 5, and new entrants  to the district. The physical must be dated within the last 12 months and filled out on the official New York State form. 

Any student who does not submit a current health examination will be scheduled to be seen by the school physician. Please call the health office at 678-8514 or email me at cgarofalo@oceansideschools.org. with any questions or concerns.
